Instructions: Please fill out the activity proposal form below. You may access any of the resources below in helping to identify the competencies that your proposed activity addresses and in developing objectives for you activity. After completing the form please meet with your mentor to review the form.  The completed form is then submitted to the Interdisciplinary Studies office for approval.
